Release of Liability
For the permission of Alexandria Zen Center to use its facilities and of the execution by others of agreements similar, the undersigned agrees that while upon the premises of Alexandria Zen Center or while using its facilities or equipment, whether at Alexandria Zen Center or at any other location for the purpose of practice or of demonstration, said premises, facilities, and equipment shall be occupied and used at the sole risk and responsibility of the undersigned, and the undersigned hereby releases Alexandria Zen Center from any and all claims for personal injury, damage, or loss of any kind or description resulting from being thereon or from such use or from the acts of any persons thereon. The under-signed further agrees to indemnify and hold harmless Alexandria Zen Center and each of its instructors, teachers, officers, directors, and members from or against any and all claims made or instituted against it or them arising out of the acts of the undersigned while upon the premises of Alexandria Zen Center or while using any of its facilities or equipment, whether at the Alexandria Zen Center or any other location for the purpose of practice or demonstration, including injury or loss to the undersigned however caused and injury or loss caused by the undersigned to any other person. By signing this agreement, the applicant* acknowledges the contagious nature of COVID-19 and voluntarily assumes the risk that the applicant* may be exposed to or infected by COVID-19 by attending events under Alexandria Zen Center, and that such exposure or infection may result in personal injury, illness, permanent disability, and death. The applicant* understands that the risk of becoming exposed to or infected by COVID-19 at a Alexandria Zen Center event may result from the actions, omissions, or negligence of the applicant* and others, including, but not limited to, Alexandria Zen Center volunteers, and program participants and their families. The applicant* voluntarily agrees to assume all of the foregoing risks and accept sole responsibility for any injury to the applicant’s* self (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that the applicant* may experience or incur in connection with the applicant’s* attendance at Alexandria Zen Center events. The applicant* hereby releases, covenant not to sue, discharge, and hold harmless Alexandria Zen Center, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. The applicant* understands and agrees that this release includes any Claims based on the actions, omissions, or negligence of Alexandria Zen Center, its employees, volunteers, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Alexandria Zen Center event.
